What is Personalised Care and Support Planning?

Personalised care and support planning is a coordinated planning process which aims to integrate the person’s experience of accessing health and social care services so that everything is contained within one joined-up plan, covering all of their health, social care and holistic wellbeing needs. The person is in the driving seat of the process, and the plan is developed with their involvement. The introduction of Personalised Care and Support Planning forms part of the NHS Long term plan.

Course Information:

This course has been developed to guide practitioners through the history and context leading to the introduction of Personalised Care and Support Planning, the values and philosophy which underpin the planning approach, and some helpful tools and strategies which practitioners can build into their work alongside people who draw upon health and social care services. This course content has been specifically developed for practitioners who work alongside people with Learning Disabilities and Autistic people.
